I am not sure how reproducible this is but a few times when I would try to use the smoke, I'd click before the equip animation was done (you can hear the pin being removed but no animation), which then does nothing. Clicking again will throw both smokes, one at your feet and another the desired direction. If anyone else can reproduce this and post a gif or something, that would be great.
