Spawning on American squad leader - unable to spawn at times?

I tested this with a friend, and we confirmed that in some circumstances your squad mates are not able to spawn on the squad leader. The leader was alive, in an open area, not under fire and not far away behind enemy lines.

Are there any conditions we need to know which prevent squad members from spawning on US SL? If not this is clearly a bug and makes lives of US players even harder as it is.
