A code issue, most likely.
As for reproducibility, this is actually the first time I noticed this issue, but most likely not the first time it has happened.

As you can see, I'm a commando with the support's machete.
I believe this happened because I started the game out as a support to finish getting it to level 16, then I switched to commando to do the same. Whilst running around looking for the last zed, I pulled out my knife and noticed it wasn't the KA-BAR that the perk usually gets.
