I think it would be fun to get some cosmetic rewards from achievements like skins, weapon camos, additional songs, etc. For now we only get a steam achievement.
For example, passing the Hell on Earth difficulty would unlock demon horns and steamy/demonic visual effects that you can equip your character with. The horns could grow as you finish more and more HoE maps.
Mr.Foster could get his tuxedo back if you own Killing Floor 1 and play -let's say- 10 hrs in total with him.
You could also unlock a golden gas mask/weapon camo if you find all Dosh Blings in a map, and so on for all characters.
